COVID-19 MvsS

COVID-19 MvsS identifies gene expression differences between severe and mild COVID-19 patients to support prognostic biomarker discovery using peripheral blood transcriptomics.

Key Features:

  • Systematic review and re-analysis: Re-analysis of seven publicly accessible peripheral blood transcriptomics datasets comprising 140 severe and 181 mild COVID-19 patients.
  • Identification of prognostic biomarkers: MCEMP1, HLA-DRA, and ETS1 were the most consistently differentially expressed genes across datasets, with increased MCEMP1 and decreased HLA-DRA detectable as early as four days before respiratory function nadir.
  • Single-cell RNA-sequencing insights: Analysis of peripheral blood mononuclear cell single-cell RNA-sequencing data localized differential expression of MCEMP1 and HLA-DRA predominantly to CD14+ cells.
  • Longitudinal monitoring: Inclusion of an independent prospective cohort with longitudinal blood transcriptomics provided temporal resolution of gene expression changes relative to disease progression.

Scientific Applications:

  • Prognostic biomarker discovery: Enables identification and validation of early prognostic biomarkers for severe COVID-19 based on peripheral blood gene expression.
  • Temporal disease monitoring: Supports temporal mapping of gene expression changes to stages of respiratory decline.
  • Immune cell–specific mechanism investigation: Facilitates study of cell-type specific immune responses by linking differential expression to CD14+ cells in peripheral blood mononuclear cells.
  • Clinical decision support research: Informs research on patient triaging and personalized treatment strategies based on transcriptomic signatures.

Methodology:

Systematic review and re-analysis of seven publicly accessible peripheral blood transcriptomics datasets, analysis of peripheral blood mononuclear cell single-cell RNA-sequencing data, and analysis of longitudinal blood transcriptomics from an independent prospective cohort.
