MENDS-on-FHIR

MENDS-on-FHIR leverages the OMOP Common Data Model and FHIR standards to create a standards-based extraction-transformation-load pipeline for national chronic disease surveillance in distributed data networks.

Key Features:

  • OMOP-to-FHIR Transformation: Uses Whistle, a JSON-to-JSON transformation language, to convert OMOP CDM Version 5.3 data into FHIR R4 V4.0.1/US Core IG V4.0.0 conformant resources.
  • Bulk FHIR Data Exchange: Employs a REST-based Bulk FHIR $export request to extract FHIR resources for population-level data exchange.
  • Standards Compliance: Produces FHIR/US Core compliant resource types with less than 1% non-compliance rate.
  • Local FHIR Server Storage: Stores transformed FHIR resources in a local FHIR server prior to extraction.

Scientific Applications:

  • Chronic Disease Surveillance: Enables interoperable data exchange and transformation for the Multi-State EHR-Based Network for Disease Surveillance distributed data network.
  • Population-Level Data Exchange: Supports standardized data element sharing required for public health surveillance use cases through interoperable FHIR resources.
  • Alternative OMOP Data Sharing: Provides a FHIR interface as an alternative mechanism for accessing and sharing OMOP CDM data.

Methodology:

Input data in OMOP CDM Version 5.3 format from a research data warehouse undergoes OMOP-to-FHIR transformation using Whistle to create FHIR R4/US Core IG conformant resources, which are stored in a local FHIR server; a REST-based Bulk FHIR $export request then extracts these resources to populate a local MENDS database.
