PESUCA
PESUCA quantifies resected peritoneal surface area (PSA) to support assessment of surgical impact and to inform intraperitoneal chemotherapy dosing for patients with peritoneal surface malignancies (PSM) undergoing cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC).
Key Features:
- Comprehensive algorithm: Employs a programming algorithm to integrate body surface area (BSA), PSA, and regional proportions for detailed PSA quantification.
- Calculation of BSA: Establishes a baseline measure for effective contact area used in subsequent PSA computations.
- Correlation to PSA: Links BSA with PSA to support accurate initial assessments of peritoneal surface area.
- Anatomical region analysis: Calculates the relative proportion of 40 distinct anatomical regions to total PSA before CRS.
- Intraoperative input: Accepts real-time input of resected proportions in the 40 anatomical regions during CRS.
- Post-surgery quantification: Determines both resected and remaining PSA after CRS.
- Regional area assessment: Provides quantitative determination of supramesocolic and inframesocolic visceral and parietal peritoneal areas before and after CRS.
Scientific Applications:
- Enhanced dosage accuracy: Quantifies remaining PSA after CRS to enable more accurate dosing adjustments for intraperitoneal chemotherapy, including HIPEC.
- Improved comparability: Enables standardized assessment of peritoneal disease extent across patients and studies to improve comparability in clinical research.
Methodology:
Proof-of-concept evaluation in 36 patients demonstrated a mean PSA reduction from 18,741 cm² before CRS to 13,611 cm² after CRS (p<0.0001).
